Covid-19 National Vaccination Certificate
What is it?
The Covid-19 National Vaccination Certificate is a document that proves citizen's vaccination against Covid-19. Through Conecte SUS, Ministry of Health of Brazil provides the Certificate in three languages: Portuguese, English and Spanish. In addition, it is possible to download it in PDF format, print and add the document to Apple or Google Wallet.
Vaccination records are included into information systems* integrated to the National Health Data Network (RNDS). Once included, these data are automatically presented in the National Digital Vaccination Card at Conecte SUS.
After completing the vaccination cycle, citizens can issue the Covid-19 National Vaccination Certificate, in the "Vacina” (Vaccine) service of the application or in the web version of Conecte SUS.
Vaccine Registration in Volunteers: volunteers who participated in trials or clinical studies can also have their doses registered in Conecte SUS and will be able to issue the Covid-19 National Vaccination Certificate. To request registration, simply go to the nearest Basic Health Unit (UBS) and require the transcription of the information provided in a document received by the research center.
For travelers: although there is no published international regulation, some countries accept the Covid-19 National Vaccination Certificate as proof of vaccination. However, this certificate is not a travel document. Before travelling, check applicable public health measures and restrictions in the destination country.
Vaccine Registration Abroad: Brazilians and foreigners can have vaccines against Covid-19 administered abroad registered, as long as the vaccines are authorized in Brazil by ANVISA - Brazilian Health Regulatory Agency. To request the registration, simply go to the nearest Basic Health Unit (UBS) and require the transcription of the information provided in an international document. Attention: the transcribed record of vaccine administered abroad is only available in the National Digital Vaccination Card, together with other vaccines administered, available in three languages (Portuguese, English and Spanish).
Who can use this service?
All citizens who have completed the vaccination cycle against Covid-19, that is, who have taken at least two doses or the single dose vaccine and have their Immunobiological Registry registered in an information system integrated into the National Health Data Network (RNDS).
Steps to perform this service?
1. Complete the vaccination cycle (depending on the immunizing agent - two doses or a single dose) at an Immunization Center or a Basic Health Unit (UBS).
a. Note: health professionals can register it in an information system or on paper. In the last case, there is a typing process, organized by the local Epidemiological Surveillance, which in some cases may be performed by the vaccinator.
b. Duration of this step: it depends on the vaccine cycle interval of the vaccine administered. To check the interval time, access the National Covid-19 Vaccine Operational Plan.
2. Through the application Conecte SUS or the web version, citizens can check whether the vaccine record has been inserted into the information system. If it does not appear in the system after 10 days from the date of vaccination, check if there was any typing problem with the health team or state or municipal health secretariats and make sure that your data transmitted to the National Health Data Network (RNDS) of the Ministry of Health.
a. Estimated waiting time: Up to 10 days.
3. Certificate Issuance:
There are two ways to issue the document in the app or in the web version of Conecte SUS.
1st: Click on the “Meu Cartão” (My Cards) icon (in the web version, this icon is in the lower right corner. In the app, it is in the center of the lower bar) >> press the “Certificado de Vacinação Covid-19” (Covid-19 Vaccine Certificate) button >> the certificate will appear on your screen automatically.

2nd: Search and click on the “Vacinas” (Vaccines) icon >> see the administered doses and open the details of the administered doses >> click on the “Covid-19 Vaccination Certificate” button >> automatically the certificate will appear on your screen.
Estimated waiting time: 15 seconds
Note: Covid -19 National Vaccination Certificate will only be enabled after the completion of the vaccination schedule. Thus, citizens should be aware of the number of doses needed for each immunizer.
How to issue the certificate through the Conecte SUS app?
Follow the steps below to download the app on your smartphone, log in using the federal government´s single access gov.br and access the vaccination campaign data until you issue your certificate. You must have a CPF (Brazilian Social Security Number) number to access the service.

What information is available in the Covid-19 National Vaccination Certificate?
The document can be issued in Portuguese, English and Spanish, and presents: person's registration data (name, date of birth, gender, CPF, nationality and mother's name); issuance date and time of the certificate; authentication data (16-digit verification code); and information on the vaccine’s doses administered (application date, vaccine name, institution responsible for manufacturing or importing the vaccine and description of dose).
QR Code
The QR Code is used to verify the authenticity of documents. The QR Code is read using the cell phone camera, which scans the code and displays the stored content. In the case of the Covid-19 National Vaccination Certificate, the QR Code serves to verify the authenticity of the document.
Document authentication
The certificate can be authenticated using the 16-digit code and/or by reading the QRCode presented in the document.
Document authentication by the QR Code reader can be performed through the “Valida Certidão” website or through the Conecte SUS app, which, through the “Valida QR Code” icon , uses the device's native camera to scan and verify the document. This service is free for citizens.
How do I know if my vaccination records have already been sent to the RNDS/Ministry of Health?
Check in the Conecte SUS app or in the web version if the vaccination information is available. If it is not available after completing the vaccination schedule, contact the health establishment responsible for administering the vaccine to verify if the data has already been entered into the system and sent to the Ministry of Health.
What to do when identifying an information error in my Certificate?
According to the General Data Protection Law, Law No. 13,709, of August 14, 2018, it is the right of the holder of personal data to obtain, at any time and upon request, the correction of incomplete, inaccurate or outdated data. The agents who collected, typed and sent the data to the Ministry of Health are responsible for correcting it.

What information is presented in the national certificate of vaccination against COVID-19?
The document can be issued in Portuguese, English and Spanish, and presents the registration data of the vaccinated person (name, date of birth, gender, CPF), date and time of issuance of the certificate, certificate authentication data, and information about the doses of administered vaccines (date of application, institution responsible for manufacturing or importing the vaccine, name of vaccine, description of dose, batch and health establishment).
Document authentication
The certificate can be authenticated using the 16-digit code and/or QRCode presented in the document. This document is valid in national territory for twelve months from the date of issue.
Document authentication can be performed by the QR Code reader of the Connect SUS application itself, using the device's native cameras, or by the “Valida Certidão”. This service is free for citizens.
