HEALTH CARE VISA (VITEM II)
DO I NEED A VITEM II?
Citizens of any country need a VITEM II, should they wish to go through health treatment in Brazil.
A VITEM II is issued for stays longer than 90 days. Should the length of stay be shorter than 90 days, you must apply for a Visitor Visa (VIVIS).
ATTENTION: Read all the instructions on this page very carefully. Visa applications which do not contain all required documents will not be accepted under any circumstance. The Consular Section may eventually ask for additional documents. Visa fees are non-refundable, even in the event of non-concession.
NECESSARY DOCUMENTS
1. Passport, valid at the moment of application. Should your passport expire or be renewed before traveling, it is possible to carry both passports – the current passport (at the date of travel) should be valid for more than six months and have at least two empty pages;
2. Electronic Visa Application Form (RER), signed and with a 3x4 cm (color) photograph. The form can be filled in online, via: https://formulario-mre.serpro.gov.br/sci/pages/web/pacomPasesWebInicial.jsf
3. Proof of income, such as bank or credit card statements for the last three months, or other documents which attest the financial means to pay for the trip;
4. Air tickets or reservation, which clearly state the date of arrival in and departure from Brazil;
5. Proof of financial means for paying for the treatment, either with the applicant's own income, health insurance that covers for the intended treatment, or international agreement;
6. Medical referral or evaluation for said treatment;
7. Cost estimate for the treatment;
8. Declaration from Brazilian doctor, clinic or hospital, in which it is assured that said treatment shall not financially burden the Brazilian public health system (SUS), with the exception of reimbursement;
9. Proof of no criminal record, apostilled and translated to Portuguese, English or Spanish;
10. Birth certificate, apostilled and translated to Portuguese, English or Spanish;
11. If the applicant is not Russian or Uzbek, proof of legal stay in one of these countries (visa, residence permit, work permit, etc.).

The appointment has to be made EXCLUSIVELY on the website and it is free of charge (here).
Schedules are MANDATORY for ALL services.
The Consular Section will not process applications that lack the required documentation, as well as those whose forms have not been entirely or correctly filled.
ATTENTION: Scans of all documents must be uploaded to the electronic form. The Consular Section reserves its right to deny any application which does not have all documents uploaded. Uploading the documents does not exempt the applicant from presenting the originals of all documents.
CONSULAR FEES
The consular fee for the VITEM II varies according to the nationality of the applicant:

* A “real-ouro” is an accounting unit for consular fees, which does not correspond to 1 real. Prices, in rubles, here.
METHODS OF PAYMENT
- by cash deposit to the account of the Embassy;
- by bank transfer
* The transfer of VISA FEES and cash deposit to the bank account of the Consular Section in Moscow should be made on time, taking into account specific bank run-times, that means, at least 3 working days BEFORE the visa application is submitted;
* Visa applications will not be processed until the payment has been credited to the bank account of the Consular Section in Moscow;
* The proof of payment must always be submitted together with the application documents.

Registering a Visa with the Brazilian Federal Police
For all visas for a stay longer than 90 days, every foreigner must seek the Federal Police, within 30 days of his/her entry in Brazil, in order to enregister and apply for the RNE (Registro Nacional de Estrangeiro - National Registration for Foreigners).

General Visa Framework (QRGV)
The General Visa Framework (Quadro Geral de Regime de Vistos - QGRV) is available on the Consular Portal website (click here). The QGRV is available only for bearers of ordinary passports, traveling on tourism or business, and bearers of diplomatic and official passports. Information on other types of visas are available on this page or by e-mail: consulado.moscou@itamaraty.gov.br
The QGRV is valid only for foreingers who wish to enter Brazil and does not reflect the situation for Brazilians who wish to enter other countries. Brazilian citizens who wish to travel to another country must consult the appropriate consular office of said country.
Visa Request Form
Other than the specific document for each type of visa, all applicants must fill out a Visa Request Form, also called RER (in Portuguese, "Recibo de Entrega de Requerimento") (click here).
Please select the option "VISA". Read and carefully follow all instructions, fill out all fields until "Biometric Data and Scanned Documents", upload all documents, then click on "Complete Application". Print out the form, sign it on the designated area and paste a 3x4 color photo.
The RER is valid for one month.

Vaccination against yellow fever
Due to a recent surge on yellow fever cases in Brazil, the Ministry of Health, according to WHO guidelines, reccomends that all travelers who wish to visit Braazil get vaccinated against yellow fever. Only one vaccination in a lifetime is enough. If the tourist is taking the first vaccination on the country of origin, it is reccomended to be done at least 10 days prior to traveling to Brazil.
International Vaccination Certificate
The International Vaccination Certificate against yellow fever is a mandatory document for international tourists hailing from endemic areas of the disease. There are two types of certificate: one with a 10-year expiration and a lifetime-expiration. If a traveler has either of them, no State can demand a new round of vaccination as a condition for entering a country. It is recommended by the WHO that travelers who still have a 10-year expiration certificate to renew them as to include a lifetime expiration.
