a) Completion instructions: - The form must be filled in online. There is no printable version available for download. - Enter complete information – please note that some fields are mandatory. - Parents' names must be entered in full (name and surname), exactly as displayed in the applicant's birth certificate. - Click on the "Complete Application" button to generate the Visa Request Form Receipt (RER). b) After the form is concluded: - Print and sign the RER; affix your photo on the indicated field. - Gather all original documents listed below. They must be presented during your interview at the Consulate, in addition to the RER. Please note: documents in digital format must be printed. Valid passport |
Must have at least two blank pages. Please note that pages reserved for amendments cannot be used for visa purposes. |

One passport photo |
The photo must be recent, against white background, in the local standard specifications (3.5 x 4.5 cm). |

Certificate of Criminal Record |
Police checks are valid for up to 3 months from the date of issue. Please note: if the applicant has lived in multiple countries within the last 12 months, a police check from each country of residence must be provided. They must also be duly legalised (contain an apostille) by the competent authority in the issuing country and, if necessary, translated by a certified translator. |

Original birth certificate |
In the absence of the birth certificate, the applicant may submit another official document that contains the parents’ full names. Please note: if the document has been issued in a country other than Slovenia, the document will have to be previously legalised (get an apostille). In case the document is not drawn up in Portuguese, English, Spanish, French, or Slovenian an official translation (sworn or qualified translation) must also be provided.
In order to avoid having to present a translation, it is recommended to submit documents in the international model, which contain information in English (e.g. birth certificate in the "Formula/Formule/Formblatt/Formulaz A" model, valid in countries of the European Union).
Attention: submitting this document is intended to prevent any discrepancies between the information contained in the visa application and the birth certificate itself. As the birth certificate will have to be presented during the mandatory registration of the visa with the Federal Police in Brazil, any inconsistent data will preclude the conclusion of this process. Thus, the presentation of the birth certificate during the visa application stage abroad is not compulsory and will not prevent the visa from being issued, but is highly recommended in order to avoid possible inconveniences. |

Valid health insurance |
The insurance must be valid in Brazil for the entirety of the applicant’s stay in Brazil. |

Documentation from employer |
Employment or service contract(s) or other documents proving the relationship with a foreign employer or client; A letter from the employer, stating the applicant’s full name and functions in the organization, as well as a statement in which the employer attest that all functions of the employee can be performed remotely in Brazil. If self-employed, the statement must be signed by the applicant attesting the ability to perform his/her professional activities remotely, through information and communication technologies; |

Proof of income |
Pay slip in a monthly amount equal to or greater to US$ 1.500 or Bank statements for the last 3 months as proof of funds in the amount of US$ 18.000. (Original contract of employment or services agreement may be asked if deemed necessary) |

Proof of payment of consular fee |
The applicant must present proof of payment at the time of attendance. The transfer must be arranged in advance and the receipt must be issued only after payment has been confirmed by the bank. Transfers will not be allowed during the service. Cell phone screenshots and receipts that show the status “in process” (v obdelavi) will not be accepted. In the field “Payment details” (namen plačila), enter the name of the applicant and the service required. Examples: John Doe – passport; José das Couves – marriage registration; Jana Vzorec – visa.
