VITEM II - Health Treatment
TEMPORARY VISA II - HEALTH TREATMENT
a) General rules
- Foreign citizens traveling to Brazil for health treatment purposes may request VITEM II for stays of more than 90 days.
- For stays of up to 90 days, please apply for a Visit Visa (VIVIS).
- Within 30 days of entry in Brazil, holders of this type of visa must register at the Federal Police (click here) and submit a copy of the visa application form provided by this Embassy.
b) Length of stay in Brazil
This visa is valid for up to 1 year.
c) Required documents
Please note that you must bring the original documents to the appointment:

Must have at least two blank pages. Please note that pages reserved for amendments cannot be used for visa purposes. |

One passport size photograph |
The photo must be recent, with white background. |

Visa application form |
Must be filled in online (click here). Please print and bring the Visa Request Form Receipt (RER) to the appointment. It is very important to state the parents’ full names, as displayed in your birth/marriage certificate. For minor children (under 18 years old), the printed RER must be signed by both parents. Please note: the Embassy will only accept applications in which the required documents on this table have been uploaded to the form. |

Police check (applicants aged 18 years old or over) |
Certificates are only valid for 3 months after issuance. Please note: If the applicant has lived in multiple countries within the last 12 months, a police check from each country must be provided. They must also be duly legalised by the respective Brazilian consular authority with jurisdiction over these countries, and translated by a sworn translator, if necessary. |

Proof of financial means for maintenance |
Bank statements for the last 3 months or credit card statements proving financial conditions for their maintenance in Brazil for the period of stay. |

Proof of financial means to pay for the treatment |
The following will be accepted: a) own resources
b) health insurance valid in Brazil that offers coverage for the specific treatment; or
c) certificate of provision of health service provided for in an international agreement.

Medical report
|
Medical indication or medical report for the treatment.

Expected costs of medical treatment |
Document issued by the institution responsible for the treatment declaring the approximate costs involved. |

Medical statement |
Statement from the responsible physician, hospital or clinic where the treatment will be carried out, ensuring that the treatment will not encumber the Brazilian Unified Health System (SUS).
