Death Certificate
Important information
a. The death of Brazilian citizens in Syria can be registered at the Embassy upon submission of a declaration from a family member or authorised local funeral service.
b. When the service is requested by a funeral director, an authorisation form signed by a family member must be submitted. The signature must be authenticated by a notary public in Brazil and the form must be accompanied by a copy of the family member's ID document.
c. The consular death certificate must subsequently be transcribed in Brazil at a civil registry office ("1º Ofício do Registro Civil") in the place of residence of the deceased or in the Federal District.
d. The issue of the document is free of charge. Copies will be charged.

Medical report containing the cause of death (causa mortis) |
This document must be presented only when this information is not present in the foreign death certificate. |

Identification document of the deceased |
Submit preferably a photo ID (expired passports can be accepted). |
