GENERAL INFORMATION:
Brazilian nationals and foreigners can only apply for Life Certificate in person, at the Consulate General in Washington.
Appointment necessary
The interested party is responsible for sending the life certificate to the paying agency, either directly or through a constituted attorney..

DOCUMENTS:
1) For Brazilians: valid Brazilian Passport or Brazilian Identity Card (RG); for non-brazilians: valid Foreign Document (with photo).
2) Consular fee:
There's a fee of US$ 5.00 per document.
Payment must be made in form U.S. Postal Money Orders payable to the “Consulate General of Brazil
NOTE: Applicants may choose to provide the Consulate with self-addressed Express Mail return envelope from the United States Postal Service (USPS) with stamps/postage in the right amount to have the certificate mailed back once it is ready (we do not accept envelopes submitted by other carriers, such as Fedex, DHL, UPS, etc). Take note of the tracking number on your USPS envelope. After the life certificate is ready, all documentation will be returned to the address on your return envelope. Use the tracking number on the USPS return envelope to check when your documentation has been mailed to you and its current status.The Consulate is not responsible for any items lost in the mail.
