CONSULAR REGISTRATION CARD
General Information
1. The Consular Registration Card is a document created to assist Brazilians living in the United States. When issuing the card, the Consulate enters the applicant’s information, including emergency contacts in the United States and Brazil, into an internal registry. This information is for the Consulate's exclusive use in cases of emergency and will not, under any circumstances, be disclosed to anyone.
2. The Consular Registration Card contains the following information: name, nationality, address in the United States, place of birth, height, photo, and signature. Although the information on the card is in English, it is not an official U.S. form of identification. This card does not replace a driver's license or passport, nor is it proof of immigration status.
3. The Consulate General of Brazil in Washington issues registration cards only to Brazilians residing in its jurisdiction (District of Columbia, Virginia, Maryland, Delaware, West Virginia, North Carolina, Ohio and Kentucky). Residents of other states must request a card from the Consulate within whose jurisdiction they reside.
4. The Consular Registration Card is issued at no cost to the applicant.

Required Documents
1. Consular Registration Card Form (Microsoft Word) (PDF) completed and signed
For minors under 18: Both parents must complete the authorization section (Page 2 of the form) and sign in the presence of a consular agent or a notary public.
2. Two forms of Brazilian identification (at least one with a photo)
Birth certificate
Marriage certificate
ID card (RG)
Passport
3. Recent 2”x2” passport style photograph
4. Proof of residence in the United States with current address:
Most recent utility bill (water, electricity, gas, internet, etc.)
Current Lease Agreement
Valid U.S. driver's license
5. For minors (under 18): valid photo ID of both parents listed on Brazilian birth certificate
Documents must be original.
Brazilian parents must present Brazilian identification (passport or RG). Non-Brazilian parents may present a passport or a U.S. driver’s license.
6. For mail-in applications only: A pre-paid Priority or Priority Express flat-rate return envelope from the U.S. Postal Service (USPS) with your address in the “TO:” section
The Consulate will use this pre-paid envelope to return your documents along with your new Consular Registration Card. Fold this envelope in half and include it with the required documents listed above if requesting the document by mail.
How to Submit a Consular Registration Card Application
1. Create an application for a Consular Registration Card (Carteira de Matrícula Consular) in the e-consular system.
2. In the e-consular system you will be asked to upload scans or photos of the required documents. The Passport Sector at the Consulate General will review your documents and, if necessary, request corrections or other documents.
3. Once your application in the e-consular system has been approved, you may choose to make an appointment at the Consulate General to bring your original documents, or send them by mail.
4. Applicants presenting their documents in person at the Consulate General (by appointment only, via the e-consular system) will receive the Consular Registration Card on the day of their appointment.

6. Mail-in applications are processed within 3 business days after receipt at the Consulate. It is highly recommended that the applicant take note of the tracking numbers on both the sending and the return envelopes before mailing in their application.
