VITEM XI - Family Reunion

The VITEM XI visa may be granted to an individual (hereby referred to as “applicant”) who is the parent, guardian, grandparent, child, grandchild, spouse, partner of a Brazilian citizen or of a temporary or permanent resident in Brazil (hereby referred to as “visa sponsor”)

- Original passport (signed, valid up to the end of Brazil trip, with at least 2 visa blank pages)

- Proof of residence in the U.S. (copy of visa or Green Card) for non-U.S. citizens
- Proof of residence in consular jurisdiction (copy of a valid driver's license or utility bill will be accepted as proof of residence)
- Visa Application Form Receipt: https://formulario-mre.serpro.gov.br/sci/pages/web/ui/#/servicos-estrangeiros
- Passport-style photo (2”x 2”, white background)
- USPS money order – see visa fees here
- Birth Certificate (original and one copy)

Birth certificate may also serve as proof of the relationship between applicant and visa sponsor (see below) if the former is the child of the latter

- FBI Background Check (issued within the past 60 days).    Minors (younger than 18 years of age at time of application) are exempt from this requirement. See Visa Requirements for Minors

- Proof of the relationship between applicant and visa sponsor (one of the following): 1) Birth Certificate (if applicant is the child of the visa sponsor), or 2) Marriage Certificate or document that proves civil union/domestic partnership (if applicant is the spouse of the visa sponsor)
- Sworn affidavit stating that the applicant’s visa sponsor either currently resides or will reside in Brazil. If your visa sponsor is your offspring (minor), click here
- Extra USPS Priority Express envelope, pre-paid and self-addressed (this will be used to mail your passport back to you)
