VITEM VII – Religious activities
- Original passport (signed, valid up to the end of Brazil trip, with at least 2 visa blank pages)
- Proof of residence in the U.S. (copy of visa or Green Card) for non-U.S. citizens
- Proof of residence in consular jurisdiction (copy of a valid driver's license or utility bill will be accepted as proof of residence)

- Passport-style photo (2”x 2”, white background)
- USPS money order – see visa fees here
- Birth Certificate (original and one copy)
- FBI Background Check (issued within the past 6 months)
- Notarized copy of Act of Establishment (“Ata de Constituição ou Estatuto Social”) of the religious institution in Brazil
- Notarized letter of invitation from the religious institution in Brazil, signed by its legal representative, assuring lodging and financial support for the
intended party and members of his/her accompanying family, and assuming full responsibility for the departure of the clergyman and his/her family from
Brazil
- Notarized letter from the Church in the United States attesting eligibility of the applicant, stating length of stay, place and type of work that will be
performed in Brazil
- Notarized certificate or diploma of theological or academic religious education indicating assignments as clergyman and school transcript of
theological studies
- Curriculum vitae (resume)
- Notarized and signed Statement of Commitment stating that the applicant will not engage in any activity within indigenous areas without the proper authorization from FUNAI (National Indian Foundation)
- Extra USPS Priority Express envelope, pre-paid and self-addressed (this will be used to mail your passport back to you)
The requirements listed above are not all-inclusive. Additional documents may be requested at the discretion of the Consular Authority.
