VITEM II - Health Treatment
(More than 90 days)
- Original passport (signed, valid up to the end of Brazil trip, with at least 2 visa blank pages)
- Proof of residence in the U.S. (copy of visa or Green Card) for non-U.S. citizens
- Proof of residence in consular jurisdiction (copy of a valid driver's license or utility bill will be accepted as proof of residence)
- Visa Application Form Receipt: https://formulario-mre.serpro.gov.br/sci/pages/web/ui/#/servicos-estrangeiros
- Passport-style photo (2”x 2”, white background)
- USPS money order - see visa fees here
- Birth Certificate (original and one copy)
- FBI Background Check (issued within the past 60 days). Minors (younger than 18 years of age at time of application) are exempt from this requirement. See Visa Requirements for Minors page.
- Evidence of income: proof of financial capability during stay in Brazil (i.e., research grant, copy of bank/credit card statements or paychecks/paystubs)
- Extra USPS Priority Express envelope, pre-paid and self-addressed (this will be used to mail your passport back to you)
The requirements listed above are not all-inclusive. Additional documents may be requested at the discretion of the Consular Authority.
