(For stay over 90 days)

• Visa Request form, signed and with recent and good quality passport size picture attached (2” x 2”), and white background
• Passport
• Proof of legal residence in the US (if applicable)
• Birth Certificate + Apostille
• Driver License
• FBI Background Check only, not older than 03 months.
• Airline ticket reservation, with estimated departure and return dates
• Return Declaration form (If you do not have flight reservation)
• Proof of financial ability (Last 03 bank statements/ savings account, or the last 6 Pay Stubs)
• Proof of financial ability to cover the treatment (personal finances or proof of health insurance with specific coverage for the specific treatment, with Medical Report from your doctor for the specific treatment
• Cost Estimate for the medical treatment from the doctor or institution in Brazil, with signature(s) notarized in Brazil
• Statement from the doctor, hospital or clinic where the treatment will be performed, assuring that your treatment will not cause any costs to the Brazilian Heath System - Sistema Unico de Saude (SUS)-, with signature(s) notarized in Brazil

United Kingdom, UK (Superior to 180 days): $215.00

All Others: $100.00

Payment: USPS Money Order or Debit Card only
