International Yellow Fever Vaccination Certificate
The National Health Surveillance Agency, ANVISA, implemented the electronic issuance of the International Certificate of Vaccination or Prophylaxis (CIVP). Anyone interested in obtaining the document must request the CIVP on the Federal Government Services Portal: https://sso.acesso.gov.br.

Electronic Request
To request the electronic certificate, the interested party must access the Federal Government services page, search for "Issuance of International Certificate of Vaccination", open a new application and fill in the registration data, attaching the following documents:
• For the first copy of the CIVP: Brazilian official identification document with photo and proof of the vaccine applied with complete data (including batch number, application date and manufacturer);
• For a second copy of the CIVP: Brazilian official identification document with photo.
After filling it out, send the request through the Services Portal system. The application is analyzed by ANVISA. If approved, the document can be printed and signed at the indicated location.
Attention! Brazil does not issue CIVP to citizens who received the vaccine in other countries.
Loss or misplacement
If the CIVP has been lost or misplaced, it is possible to ask ANVISA to verify whether the certificate has been registered or not. If the registration is not found in the system, the applicant will be sent guidance on the need to enter the national proof of vaccination issued in Brazil.
