Passports for minors under 18
The Consulate of Brazil in Chicago issues passports to Brazilian minors under 18 upon presenting all original supporting documents plus the consent form signed by both parents or legal guardians. Copies are not accepted. All passport applications must undergo previous document screening, performed online through the E-Consular platform.
Minor Individuals under 18 must complete THREE different forms:
- formulário RER (Application Form Receipt);
- formulário de autorização para concessão de passaporte para menor; (Consent Form for Minors with travel authorization), and;
- formulário online da plataforma E-Consular (E-Consular Form).
The Consulate will only analyze passport applications after all forms are properly completed and the corroborating documentation uploaded through the E-Consular platform.
STEP-BY-STEP
1. Gather all of the required supporting documents. See the table below.
2. Select how you want to apply (in person or by mail - only for Brazilians residing within our jurisdiction).
3. Access the E-Consular platform and upload the complete list of required documents for screening. If the document is too long and has many pages, upload only the pages with the most relevant information.
4. Money Order from the United States Postal Service.
5. Only after being notified that documents were successfully verified and approved, you may forward the application to the Consulate by mail or appear in person on the scheduled date and time
ATTENTION: Be aware that you must present the originals of the same documents previously uploaded to the E-Consular platform. If you choose by mail, the originals must be included in the envelope, or if you choose in person, documents must be presented to the consular agent.
Read the following instructions very carefully. The table below lists the documents required to apply for a Brazilian passport for minors under 18. The consulate may request further supporting documents if deemed necessary.

IMPORTANT: All documents issued at Brazilian Consulates abroad now require the presentation of the CPF number for the document holder. Please have your CPF number with you when submitting a request through E-Consular.

Consent to have a passport + authorization to travel for minors under 18 |
The Consent Form must be completed and signed by both parents. The Consent Form for a minor is mandatory. Whether or not the minor's travel authorization is included in the passport is optional. To learn more about travel authorization (Consent) for minors under 18, please read the instructions below. Parents' signatures on the Consent Form for children under 18 can be certified in person at the Consulate at the time of the appointment for passport application. Parents must bring the form completed, but not signed, along with the minors' and parents' IDs. If one of the parents is not present (or applying by mail), the signature (s) must be notarized in person by a notary public. The Notary must then be certified by the Secretary of State or County Clerk through an Apostille. Note: if the parent’s ID bears a name different from the one on the minor's birth certificate, a document proving the name change (full-content marriage or divorce certificate) must be presented. If one of the parents is in Brazil, he/she must sign the passport/travel consent form and notarize it in person at a notary's office (Cartório). If there is a court sentence granting custody of the child to one parent, both parents must sign the passport consent and present their original IDs unless the ruling specifically exempts one parent from passport/travel consents. The court ruling presented must be dated within 6 months of the passport application date. If, in the custody sentence, there are restrictions regarding passport issuance consent or travel authorization of the minor, a Court Order that specifically authorizes the passport issuance will be required. In the case of an American court document, the judgment must be an original document or True Copy and bear a recent (less than 6 months) Court date stamp. A death certificate must be presented (original or true copy) in case of a deceased parent. |

The only payment method accepted is Money Order issued by The United States Postal Service. Personal checks, Credit or debit cards, and cash are not accepted. The Money Order must be presented to the consular agent with the application documents. When applying by mail, the Money Order should be mailed in the same envelope as the documents. |

For mail-in applications, include a prepaid, self-addressed Priority or Express return envelope from the USPS (United States Postal Service). The Consulate highly recommends getting the tracking number. ATTENTION: do not use any other mail carrier. The failure to do so may result in delays. The envelope can also be presented by applicants who apply in person and wish to have the passport delivered to their homes. Applicants who wish to pick up their passports at the Consulate do not need to bring an envelope. |
PROCESSING TIMES
After the application documents are uploaded to E-Consular, the Consulate requires a period of five (5) business days to validate the application and enable scheduling. The passport is issued within five (5) business days from the presentation of the required documentation at the Consulate. The applicant can either pick up the passport in person or drop off a prepaid USPS envelope to have the document mailed to their address when ready.
For applications by mail, the processing time is ten (10) business days from the date the consulate receives the complete documentation. Incomplete or incorrect documentation will not be processed and will be returned to the sender via the return envelope.

CONSULAR FEE
The only accepted payment method is Money Order issued by the United States Postal Service. Personal checks, credit/debit cards or cash are not accepted. Failure to observe this requirement will result in processing time delay.

CONSENT INCLUDED IN THE PASSPORT
See below a sample of the passport consent form with the option of including travel authorization for a minor under 18 bearing the Secretary of State’s Certificate (Apostille), which verifies the notary public. The validity of this consent is 90 (ninety) days from the date of the notary’s signature.
- Entering the travel authorization included into the minor's passport is optional in lieu of the consent form, which is mandatory.
- If the parents choose not to authorize how the child may travel, the authorization field on the passport consent form should be left blank.
- The form may only be used for including the travel authorization in passports issued by the Consulate of Brazil in Chicago.
- The validity of the consent/authorization included in the passport is at the parents' discretion, with the maximum validity being the one of the passport. The consent/authorization can only be canceled by requesting a passport.
- Without written authorization in the passport, the minor can only leave Brazil accompanied by both parents or upon presentation of written authorization from the parent who is not accompanying them (duly notarized and certified by the Secretary of State) or upon Court Authorization.
