Lost, stolen or damaged passport
The Consulate of Brazil in Chicago issues passports in place of a lost, stolen, or damaged passport; upon presenting all original supporting documents. Copies are not accepted. All passport applications must undergo previous document screening, performed online through the E-Consular Platform.
For application, you must complete at least THREE different forms:
- formulário RER (Application Form Receipt);
- formulário de comunicação de extravio, furto ou passaporte danificado (Lost, stolen or damaged passport form) and;
- formulário online da plataforma E-Consular (E-Consular Form).
If the passport to be replaced is that of a minor, an extra form must also be submitted (uploaded):
- formulário de autorização para concessão de passaporte para menor (Consent Form for Minors with travel authorization).
The Consulate will only analyze passport applications after all forms are properly completed and the corroborating documents uploaded through the E-Consular platform.
REQUIRED DOCUMENTS
IMPORTANT: All documents issued at Brazilian Consulates abroad now require the presentation of the CPF number for the document holder. Please have your CPF number with you when submitting a request through E-Consular.

A US passport-size photo, 2x2 inches, taken less than 6 months before submitting the application. Photos already used in previous passports will not be accepted.

RG, Brazilian driver's License, even if expired, Brazilian Worker's History Booklet. If you do not have a valid Brazilian ID, original or certified copies of valid foreign documents will be accepted (e.g., valid driver's license, State ID, passport) as long as they were issued under the same name as the Brazilian document. In case of original or certified copy of an ID with no validity, the photo must be recent enough to allow identification. Original or certified copies of IDs issued over 20 years ago will not be accepted. Original or certified copies of IDs issued when the applicant was a minor are accepted only if issued less than 10 years ago. |

Money Order issued by the USPS |
The only payment method accepted is Money Order issued by The United States Postal Service. Personal checks, Credit or debit cards, and cash are not accepted. The Money Order must be presented to the consular agent with the application documents. When applying by mail, the Money Order should be mailed in the same envelope as the documents. |

For mail-in applications, include a prepaid, self-addressed Priority, or Express return envelope from the USPS (United States Postal Service). The Consulate highly recommends getting the tracking number. ATTENTION: do not use any other mail carrier. The failure to do so may result in delays. The envelope can also be presented by applicants who apply in person and wish to have the passport delivered to their homes. Applicants who wish to pick up their passports at the Consulate do not need to bring an envelope. |
When the damage does not impair the identification of the passport holder, please follow the guidelines for passports for individuals 18 and older or for minors under 18, where applicable.
When the damage makes it impossible to identify the passport holder, follow instructions for lost or stolen passports.
PROCESSING TIMES
After the application documents are uploaded to E-Consular, the Consulate requires a period of five (5) business days to validate the application and enable scheduling. The passport is issued within five (5) business days from presenting the required documentation at the Consulate. The applicant can either pick up the passport in person or drop off a pre-paid USPS envelope to have the document mailed to their address when ready.
For applications by mail, the processing time is ten (10) business days from the date the Consulate receives the complete documentation. Incomplete or incorrect documentation will not be processed and will be returned to the sender via the return envelope.

CONSULAR FEE
The only accepted payment method is Money Order issued by the United States Postal Service. Personal checks, credit/debit cards or cash are not accepted. Failure to observe this requirement will result in processing time delay.
