Visa for Minors
In addition to the documents listed at General Requirements please send bring the following documents to the Consulate:
1. Application form signed by both parents or legal guardian(s).
2. Notarized letter of consent (see model) signed by both parents or legal guardian(s).
The Consulate General of Atlanta will not issue a visa under any circumstance for minors under 18 without the consent form. The form must be signed by both parents and the signatures must be authenticated by a notary public of this Consulate’s jurisdiction (GA, TN, AL, SC, MS)The parents’ signature in the form, even if notarized, must be similar to the signature in the identification document presented.
3. Original ID of the parent who is applying for the minor's visa.
4. Original and copy of the minor's birth certificate.
5. Other documents that prove guardianship (e.g. court order certified within the last year).
6. The child of a Brazilian citizen has the right to Brazilian citizenship and passport. For information regarding how to register your child as a Brazilian citizen, click here. If you do not wish to apply for the Brazilian citizenship of your child, and would like, instead, to obtain a visa, you must bring a notarized copy of the following "Statement".
SPECIAL REQUIREMENTS FOR MINORS
Any person under 18 years old is considered "minor" in Brazil.

- Minors Traveling Without Both Parents to Brazil
- Children of Brazilian Citizens

AUTHORIZATION FOR ISSUANCE OF A BRAZILIAN VISA
- Applicants under 18 years old are considered MINORS and MUST NOT SIGN THEIR VISA REQUEST FORMS.
- Both parents must fill out and sign the Authorization Form for persons under 18 years old traveling to Brazil and the minor's visa request form. Their signatures on the authorization form and the visa request form must be notarized.
- When filling in the application form it is very important to state the parents’ full names, as stated in their birth/marriage certificate.
- We advise that the Brazilian visa application form signed by the minor itself will be declined and the fee will not be returned.
- Original or certified copy of minor applicant's birth certificate must be presented.
Important! Minors’ visas will not be issued without consent of both parents, except if:
- One of the parents is deceased, in which case the death certificate (original, certified or notarized copy) must be submitted; OR
- One of the parent’s is not listed on the birth certificate; OR
- Court Order or Divorce Decree granting not only full custody but exclusive parental rights to one of the parents or legal guardian(s). The original or notarized copy of the complete document must be presented along with the other requirements.

MINORS TRAVELING WITHOUT BOTH PARENTS TO BRAZIL
It is recommended that minors who are traveling unaccompanied, or under responsibility of only one parent or third party to Brazil to carry a certified copy of the birth certificate AND a notarized Authorization Form for persons under 18 years old traveling to Brazil signed by both parents or legal guardian.

CHILDREN OF BRAZILIAN CITIZENS
Children of Brazilian citizens can only apply for a visa if they have not yet been issued a Brazilian birth certificate and therefore are not eligible for a Brazilian passport. In that case, the Brazilian parent(s) must attach a notarized* Declaration of Non-Citizenship stating, under the penalties of the law, that the child was not registered in any Consulate or in Brazil and, therefore, does not hold Brazilian citizenship.
* NOTE: If the Brazilian parent sign it in front of our attendants, the notarization is dismissed. Foreing parent(s) must notarize their signatures on the form prior to submitting it to the Consulate.
Brazilian citizens are advised to register their children at the Consulate (Brazilian birth certificate) to guarantee their Brazilian citizenship and safeguard any rights in the future.
Children of Brazilian citizens to whom a visa was issued must have it cancelled after they acquire Brazilian citizenship.
