Travel authorization for minors in the new passport
TRAVEL AUTHORIZATION FOR MINORS IN THE NEW PASSPORT
Parents may include the "Travel Authorization" inthe passport, it is optional and free. It will allow the minor to leave Brazil or with one of the parents indistinctly, accompanied by third parties or unaccompanied. When applying for a passport on behalf of your minor children. This will avoid the need to request single authorization documents for each minor's journey.
The note of travel authorization can only be made at the time of issuance of the passport.
Both parents must sign the Authorization Form to a public notary if the parents will not attend the Consulate. If a parent lives in Brazil, they must recognize the signature in a registry office. In this case, a certified copy of the identification document will be accepted instead of the original document, but the original authorization form must be presented, with the original seal of the registry office authenticating the signature of the father or mother (for authenticity, i.e. attending the registry office in person) resident in Brazil.
Choose a form below that must be completed and signed by applicants, without abbreviations, typed or handwritten in form letter, with blue or black pen:
- With permission to travel in the company of a parent
- With permission to travel in the company of a third party
- With permission to travel unaccompanied
- No travel authorization
